최근 간암을 포함한 여러 암종에서 조직 기반 분석만으로는 반영하기 어려운 종양의 이질성, 치료 반응 예측, 실시간 모니터링의 필요성이 강조되면서, 이를 보완할 수 있는 액체 생검 (liquid biopsy)의 일종인 ctDNA (circulating tumor DNA) 분석이 주목을 받고 있습니다. 특히 면역항암제 시대가 본격화되면서 치료 반응을 조기에 예측하고 예후를 판단할 수 있는 비침습적 바이오마커에 대한 임상적 요구도 더욱 커지고 있습니다.

침윤성 간세포암에 대한 기존 연구들은 주로 CT 또는 MRI 영상을 통해 종양의 형태를 관찰하거나 환자의 나이와 간 기능 등 임상 정보만을 이용해 예후를 추정한 반면, 영상, 임상, 그리고 유전체 정보를 통합적으로 분석한 연구는 거의 없었습니다. 우리 연구팀은 아테졸리주맙과 베바시주맙 병합치료를 받은 307명의 진행성 간세포암 환자에 대해 영상 이미지를 기반으로 종양 형태를 4가지 유형으로 분류하였습니다.

항암 바이러스는 정상 세포에는 영향을 미치지 않으면서 암세포를 선택적으로 감염시키고 파괴시키는 항암치료제입니다. 레오바이러스는 인간의 상부 호흡기 및 위장관에서 무증상 또는 경미한 감염을 유발하는 항암 바이러스의 한 종류입니다. 레오바이러스는 종양내 또는 정맥 주사를 통해 전임상과 임상 시험에서 평가되었지만, 이러한 투여 경로는 최적의 치료 효능을 유도하기에는 한계가 있습니다.

Stimulator of interferon gene (STING) agonist는 종양을 인지하여 종양미세환경의 면역 세포를 활성화시킴으로써, 전임상 연구에서 큰 가능성을 보여주었습니다. 하지만 STING agonist는 제한된 전신 전달의 문제점을 가지고 있습니다. 여기에서 우리 연구 팀은 STING agonist가 담지된 양전하 리포좀(PoSTING)을 설계하여 효율적인 전신 전달 및 종양을 표적 하고자 하였습니다.
