소아암 환자의 5년 생존율은 90%에 이르지만, 화학치료를 받은 생존자 중 단 9%만이 생식 능력을 유지할 수 있습니다. 따라서 남아 소아암 환자는 화학치료 전에 정소 조직을 냉동 보관하여 생식 능력을 보존하는 것이 필요합니다. 그러나 현재 기술로는 시험관에서 정자를 생성하거나, 화학치료 후 자가이식을 통한 생식 능력 복원이 실현되지 못하고 있습니다.

방광염(Urinary tract infection, UTI)은 성인 여성의 60%가 일생동안 한 번 이상은 겪을 정도로 흔한 질병이며, 그 중 20-30%의 환자가 재발성 방광염 (recurrent UTI, rUTI)을 겪습니다. 재발의 원인에 대해서는 명확하게 밝혀진 기전이 없어 예방요법으로 장기적인 항생제 복용 (daily prophylaxis)이 사용되고 있지만, 이는 내성균 발생과 환자의 장내 미생물군에 영향을 미칠 수 있다는 제약이 있습니다.

1. 논문관련 분야의 소개, 동향, 전망을 설명, 연구과정에서 생긴 에피소드 제가 연구하고 있는 폐(Lung)는 외부 환경과 항상 접촉하고 있는 기관으로, 공기를 통한 바이러스와 박테리아 같은 다양한 pathogen 혹은 chemical들에 의해서 지속적으로 손상을 입고 있습니다. 이번 코로나 바이러스로 인한 판데믹 상황에서도 알 수 있듯이 폐는 이러한 감염의 제 1차적 공격을 받는 기관일 뿐 아니라, 이로 인한 폐 손상은 생명과도 직결되는 아주 중요한 기관입니다. 다행히도 폐에는 성체 줄기세포 (stem cells)가 존재하여 상피 세포 손상 후 재생 능력을 가지고 있습니다. 저의 연구는 폐가 어떻게 재생되는지, 특히 폐 세포 손상 후 면역반응에 의한 줄기세포 및 주변 세포의 조절에 관한 질문에서 출발하였습니다. 이번 발표된 논문은 산소 교환에 중요한 기능을 하는 폐포 (alveolar) 손상을 치유하기위하여 세포 손상을 인지한 면역세포가 어떻게 줄기세포의 활성과 분화 능력을 조절할 수 있는지에 관한 하나의 조절 기작을 제시한 연구입니다. 세포 손상 이후 대식세포의 (Macrophage)에서 분비되는 인터루킨-1β (IL-1β)에 의한 염증반응 (Inflammation)이 상기도부 (Airways)에 존재하는 ciliated cells 의 notch ligand 발현은 감소시킴을 확인하였습니다. 이러한 결과로 ciliated cells 옆에 분포하고 있는 secretory cells의 Notch activity가 감소하게 되고, 이를 통하여 secretory cells은 폐포 재생에 중요한 역할을 하는 폐포 제 2형 (AT2) 세포로 분화 능력을 획득을 하게 된다는 것을 확인하였습니다 (그림 1). 이 연구를 위하여 다양한 생쥐 모델을 이용하였으며, 또한 feeder-free한 조건에서 줄기세포를 배양할 수 있는 human lung organoids를 처음으로 수립하였습니다. 저희의 이번 연구 결과는 작년 발표한 논문과 더불어 어떻게 염증반응이 폐 재생 과정을 조절하는지, 그리고 이런 과정이 어떻게 인간 폐 질환의 발병과 연관이 되는지를 밝히는데 도움이 되리라 생각됩니다. Adult Lung organoids는 intestine과 달리 chemically-defined된 Feeder-free condition에서 키우는 방법이 알려져 있지 않았는데, 여러가지 cytokine을 처리해 보다 1달이 지나도 자라지 않아서 plate를 버릴까 하다가 냉동고에 있던 Fgf7을 넣어주었을 때 갑자기 organoids가 형성 되는 것을 보고 엄청 희열을 느꼈던 2017
