1. 논문관련 분야의 소개, 동향, 전망을 설명, 연구과정에서 생긴 에피소드 요즘 뉴스나 신문기사에도 많이 언급되고 있는 렘수면행동장애(REM sleep behavior disorder, RBD)는 렘수면 동안 정상적으로 발생하는 골격근의 무긴장증(atonia)이 소실되어, 잠꼬대같이 꿈의 내용과 관련된 움직임이 나타나는 질환입니다. 렘수면행동장애가 최근 많은 주목을 받는 이유는 바로 파킨슨병이나 루이소체 치매와 같은 시누클레인병증(synucleinopathy)의 확실한 전구단계이기 때문인데요. 10년 이상의 장기 추적 관찰에서 80%이상의 렘수면행동장애 환자들이 파킨슨병이나 루이소체 치매로 전환된다는 사실이 밝혀졌습니다. 하지만, 모든 렘수면행동장애 환자들이 시누클레인병증으로 진행하는 것은 아니며, 또한 진행한 환자들 내에서도 진행속도가 달라 어떤 환자는 1-2년 이내에 퇴행성질환으로 진행하지만 어떤 환자는 10년 이상 지난 후에나 퇴행성질환으로 진행합니다. 따라서, 최근에는 이러한 진행을 정확히 예측하는 imaging biomarker를 찾는 연구가 활발히 진행되고 있으며 이러한 imaging biomarker 중 대표적인 것이 바로 FDG-PET 영상을 이용한 파킨슨병과 관련된 뇌대사패턴(PDRP), 렘수면행동장애와 관련된 뇌대사패턴 (RBDRP) 입니다. 하지만 기존 연구들은 모두 cross-sectional study design으로 렘수면행동장애 환자들에서 이러한 뇌대사패턴들이 시간이 경과하면서 어떻게 변하는지를 제시하지는 못했습니다. 본 연구는 렘수면행동장애 환자군과 렘수면행동장애가 선행하였던 파킨슨병 환자군을 각각 정상군과 비교하여 PDRP와 RBDRP를 추출하였고 이러한 패턴들이 2년 및 4년째 추적 영상을 통해 어떻게 변화하는지를 분석하였습니다. 흥미롭게도 렘수면행동장애 환자에서 보이는 RBDRP는 시간이 지남에 따라 breaking down 되면서 점차 PDRP로 build up 되었고 주로 premotor areas의 metabolic alteration에 의해서 매개되었습니다. 이러한 결과들은 렘수면행동장애 단계에서 이미 신경퇴행이 진행중이며, 뇌대사능도 이에 따라 변화한다는 것을 잘 보여주었습니다. 사실 처음에는 결과를 해석하는 데 있어 많은 어려움이 있었습니다. 하지만 결과를 제대로 해석하고 설명하기 위해 연구팀 내에서의 열띤 토론이 결국 좋은 결과로 이어졌다고 생각합니다. 또한 훌륭한 리뷰어들을 만나 비록 어렵지만 건설적인 커멘
